Required Forms

 If you are applying to be a first-year noncom, you need to submit the following:

Online references from three adults who know you well and are NOT related to you. Provide this link to the people who you are asking to complete references on your behalf:

Once you are accepted, you will need to submit the following forms:

  1. You may receive an email message from Trusted Employees asking you to submit an online background check release.

  2. Trading card information form

  3. A high resolution picture of yourself that shows your face clearly. This picture will be used on your trading card. (Send this picture to Mark via email.)

  4. Camp Service Details form 

  5. Medical form. You may complete this online by logging into your online Active account.


If you are returning for your second or third summer, you do not need to submit written references, although you should still list references on your application. We need the following forms from you:
